Breast reconstruction - free flap

When a mastectomy for breast cancer is necessary, a patient may wish to explore their options for an immediate or delayed reconstruction. This can be achieved using an implant-based technique; a pedicled flap such as a latissimus dorsi (LD) flap or a free flap such as a DIEP, TUG or SGAP.

Free flap immediate reconstructions are carried out as a combined procedure with both a Consultant Oncoplastic Breast Surgeon and a Consultant Plastic Surgeon skilled in the microvascular anastomosis necessary to reconnect a free flap to a blood supply near the breast.

Free flap surgery is the most extensive surgery for breast reconstruction but has the advantage of using only natural tissue. That tissue is skin and subcutaneous fat taken from the lower abdomen (DIEP flap); the inner thigh (TUG flap) or the buttock (SGAP flap).
